positions.
Altoona & State College Offices
On-Call Weekend RN
On-Call Weekend RN responsible for covering entire county service area.
Works Friday through Sunday 4:30 PM- 8 AM. Completes all applicable evaluation, assessment and
paperwork. Conducts visits to patients and communicates with physicians and case manager. Completes,
maintains and submits accurate and relevant clinical notes regarding patient's condition and care
given. Records pain/symptom management changes/outcomes as appropriate. Works with patient and
family/caregiver to educate and administer self care techniques.
Team Manager
The Team Manager is responsible for ensuring that patient care is coordinated and managed appropriately. The Team Manager is responsible for ensuring that care and
services are delivered appropriately and for the supervision of clinical personnel.
Qualifications: Registered nurse with current licensure to practice professional nursing in the state. Graduate of NLN accredited school of nursing with a BSN is preferred.
Previous experience in hospice/home care setting and with two (2) years management or supervisory experience preferred. Proven ability to work within the interdisciplinary setting.
Complies with accepted professional standards and practice. Has excellent observation, good nursing judgement and communication skills. Understands hospice philosophy and issues of death/dying.
Understands principles of pain/symptom management.
Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
Complies and maintains current CPR certification. Demonstrates excellent observation, verbal and written communication skills.
Carnegie Office
Home Health Aides
The Home Health Aide works under the supervision of a registered nurse.
Duties include but are not limited to daily hygiene, feeding and taking and recording
medical information of patient. Duties also include respite care for patient
family/care givers, and performing routine household chores as needed. Applicants
must be Certified Nursing Assistants, and possess a current CPR certification.
Qualifications: Must be at least 18 years of age. Understanding of hospice philosophy, compassion
and ability to work with little supervision necessary. Must be able to read and follow
written instructions.

Medical Social Worker
In this position the MSW will -assess the Psychosocial status of patients, families and
caregivers related to the patients terminal illness -Carries out social evaluations and plans intervention
based on evaluation findings -Provides information and referral services for organization patients and
families/caregivers regarding practical and environmental needs -Serves as liaison between patients and
families/caregivers and community agencies -Maintains collaborative relationships with organization personnel
to support patient care -Participates in the development of the plan of care and attends regularly scheduled
interdisciplinary group meetings -Assists physician and other team members in understanding significant social
and emotional factors related to health problems and death/dying issues -Participates in discharge planning
when needed. Applicants must be graduate of a masters program in social work accredited by the Council on Social Work Education.
Philadelphia Office
