The Hospice Interdisciplinary Team
RN Case Managers
An RN Case Manager takes charge of each patient. This means that she will be chiefly responsible for designing the nursing care plan and oversees that continuity in patient care is followed while she is off. Our hospice nurses provide highly skilled, compassionate interactions with patients and their families - comfort measures, teaching, medication, and so much more.
