How do Hospice services start?
You contact your attending physician to discuss if services are appropriate for you.
If you wish, you may also contact a hospice representative to have any of your questions answered.
If hospice is right for you, a hospice representative will meet with you and your family for admission to hospice services. Your services will begin once a plan of care is developed by
you, your attending physician and the Hospice Interdisciplinary Team that is specific to your needs.
